Once you have talked with your senior pastor and they are in full support of your desire to serve with FMI, please complete the following form. Someone from FMI will then contact you as soon as possible to dialogue with you further.

Please Note: FMI only sends personnel from U.S. Foursquare churches to serve outside the U.S. in Foursquare ministry. If you are not a U.S. citizen or are not part of a U.S. Foursquare church, please contact the Foursquare church within your country, or the Foursquare regional office in your geographic area regarding ways in which you can serve with Foursquare. Also, if you are not a U.S. citizen or are not part of a U.S. Foursquare church and you desire to serve at one of our WorldBases, please feel free to contact them directly.
